30-day readmission rates are available to our subscribers via our online reporting tool (HHIC Online Reports^®)!

 

All readmission reports are based on 3M's Potentially Preventable Readmissions (PPRs). PPRs identify return hospitalizations that may have resulted from the process of care and treatment or lack of post admission follow-up rather than unrelated events that occur post admission.The readmission data/reports allow our hospitals to monitor and compare their 30 day readmission rates, by condition, across all Hawaii hospitals.
